I have been a Registered Nurse for over 35 years. I have worked in various settings including the Neonatal Intensive Care Unit and Paediatrics to surgical care, then to the community as a visiting nurse. I wrote care plans for palliative patients and their families. I implemented these care plans and followed up with other caregivers in the home. I worked as a Care Coordinator as well. I have also worked in Long Term Care providing palliative care their also. As a certified Willow End of Life Educator, I have access to materials to help create supreme advanced care documents. These tools can be used at any age. Legacy letters and other documents can be created leaving a lifetime of information to loved ones and beautiful words that will never be forgotten.
